Why Neck and Shoulder Pain Keeps Coming Back — In Ema's Words

Shoulder and neck pain most commonly comes from posture, but also from injury and repetitive strain. The most common clients I see are office workers, people who work at screens all day, and drivers. My old business was actually called "Treat Me at the Office" because this was such a significant part of what I did. Modern life means we are sitting down a lot and internally rotating our shoulders forward. Over time this creates chronic tension in the upper trapezius and the muscles around the top of the shoulder that very few treatments actually resolve.

When I assess a neck and shoulder client I am looking at the upper cervical spine, the thoracic spine, and the muscles around the shoulder girdle. The pain might feel like it is in one spot but the cause is often coming from a different area entirely. I start with a bit of heat to relax the soft tissues, then myofascial release to free up any adhesions, then trigger point work in the upper trapezius and surrounding muscles. These trigger points are extremely common in office workers and they refer pain in patterns that can feel like headaches, toothache, or even arm pain.

I have treated a lot of frozen shoulder, impingement and rotator cuff problems over the years. One client who stands out is a lady who had been a CAD designer for most of her working life, always at a screen. She had neck pain for going on 20 years. After a course of 6 treatments she told me she had never had as much pain relief in all that time. She is still a long-term client but she no longer comes for neck and shoulder treatment. We now do full body work to help her manage some other long-term conditions. That progression, from pain relief to maintenance to general wellbeing, is what I see with a lot of long-term clients.

A common connection people do not expect is between neck tension and headaches and migraines. A lot of headaches originate from trigger points in the suboccipital muscles at the base of the skull, the upper trapezius, and the sternocleidomastoid. When I treat these areas the headache relief can be quite rapid. I also do TMJ release for the jaw in some cases, because jaw tension and neck tension feed each other. This is not always done at the first treatment as it involves intraoral work, but for clients with chronic headaches it can make a significant difference.

One simple thing I tell almost every neck and shoulder client: roll your shoulders back and let them sit down the side of your body. Not pulled back militarily, just back and down so they are aligned properly. It sounds too simple but most people with chronic neck tension are carrying their shoulders rolled forward all day without realising it. That one postural correction, done consistently, makes a real difference between sessions.

What to Expect at Your Appointment

Every treatment begins with a brief orthopaedic assessment to identify exactly what is causing your neck and shoulder pain. No two treatments are the same.

⏱ Durations

60 or 90 minutes. For complex or long-term neck and shoulder problems, 90 minutes is often recommended for the first session.

🛏 Position and comfort

You will be comfortably positioned on the treatment table, well covered with towels throughout. Only the area being worked on will be exposed at any time.

👗 What to wear

Loose, comfortable clothing works best. For neck and shoulder treatment you may be asked to remove your top – you will always be well covered with towels throughout.

🌿 The environment

Our clinic sits on the edge of a woodland park, with natural bird song. A calm, unhurried setting where you can focus entirely on your recovery.

📋 After your treatment

Your care does not end when you leave. You will receive specific stretches, postural corrections, ergonomics advice and a personalised rehab plan to take home.

Can you help with frozen shoulder?
