I often get clients who come in and tell me the doctor has said it is sciatica. My first job is to work out whether that is true sciatica coming from the spine, or whether it is actually piriformis syndrome. The symptoms are almost identical. You get pain, numbness and tingling radiating from the lower back or buttock down the leg. The difference is in the source. True sciatica originates from nerve compression at the spine, often from a herniated disc. Piriformis syndrome is caused by the piriformis muscle in the buttock compressing the sciatic nerve. Piriformis syndrome is probably the easier of the two to treat with massage, but you still have to build up to it carefully. The piriformis is a deep gluteal muscle and if you stick an elbow straight into it the client will be in agony and the nervous system will lock down. You have to earn the right to work deep.
I also check posture with crutches or walking aids if a client is using them, because the way people hold and use a walking aid can actually contribute to or worsen the piriformis tightening. This is something that often gets completely overlooked.
For both true sciatica and piriformis syndrome, the rehab is just as important as the treatment. I give specific stretches for the piriformis and the surrounding muscles, and postural guidance for how to sit, stand and move without aggravating the nerve. Those who do their exercises consistently get better faster. That is true across every condition I treat but it is particularly noticeable with sciatica.

What to Expect at Your Appointment
Every treatment begins with a brief orthopaedic assessment to identify exactly what is causing your sciatica. No two treatments are the same.
⏱ Durations
60 or 90 minutes, from £50 and £70. For new clients with complex or long-term sciatica, 90 minutes is often recommended for the first session.
🛏 Position and comfort
You will be comfortably positioned on the treatment table, well covered with towels throughout. Ema will adjust your position as needed to access the piriformis and surrounding muscles effectively.
👗 What to wear
You will be asked to undress to your underwear for most sciatica treatments. Loose, comfortable clothing to change into afterwards works best.

Piriformis syndrome
